How Physical Therapy Supports Recovery After Fractures

July 15, 2026 by Dr. Matt Siewert, PT

A broken bone is only the beginning of the healing process. Whether you have experienced a fracture from a fall, sports injury, motor vehicle accident, or another traumatic event, recovery does not end once the bone has healed. After weeks of immobilization in a cast, splint, or brace, many people are left with stiffness, weakness, pain, and difficulty returning to normal activities.

Physical therapy plays a vital role in helping patients recover safely after a fracture. At OrthoRehab, our physical therapists develop personalized rehabilitation programs that restore strength, improve mobility, reduce pain, and rebuild confidence. Every treatment plan is designed around the location of the fracture, the type of treatment received, and your personal goals so you can safely return to work, sports, and everyday life.

Understanding Fracture Recovery

Fractures occur when a bone breaks because of excessive force. Some fractures heal with casting or bracing, while others require surgery to stabilize the bone with plates, screws, rods, or pins. Although the bone gradually repairs itself, the surrounding muscles, joints, tendons, and ligaments are also affected during the recovery process.

Immobilization protects the healing bone, but it also leads to muscle weakness, joint stiffness, reduced flexibility, and loss of coordination. Even after the bone has healed, these changes can make walking, lifting, reaching, or participating in recreational activities difficult.

Physical therapy addresses these challenges by restoring normal movement and helping the body function efficiently again.

Managing Pain and Swelling

Pain and swelling are common after a fracture and may continue even after the bone begins to heal. Persistent swelling can limit movement, delay muscle activation, and make daily activities uncomfortable.

At OrthoRehab, physical therapists use evidence-based treatment techniques to reduce swelling and manage pain throughout rehabilitation. Gentle movement, manual therapy, therapeutic exercise, and education help improve circulation while supporting the healing process.

As pain and swelling decrease, patients are often able to move more comfortably and progress through rehabilitation with greater confidence.

Restoring Joint Mobility

Joints near the fracture often become stiff after weeks of limited movement. A wrist fracture may reduce hand motion. An ankle fracture may limit walking. A shoulder fracture can make reaching overhead difficult.

Physical therapy includes carefully progressed mobility exercises that restore joint flexibility while respecting healing tissues. Therapists guide patients through safe movements that gradually improve range of motion without placing unnecessary stress on the recovering bone.

Improving mobility early helps prevent long-term stiffness and prepares the body for strengthening exercises later in recovery.

Rebuilding Muscle Strength

Muscles begin losing strength surprisingly quickly during periods of immobilization. Even healthy individuals may notice significant weakness after only a few weeks in a cast or brace.

Physical therapy restores strength through progressive exercise programs designed for each stage of healing. Early rehabilitation focuses on safely activating weakened muscles before gradually advancing to resistance exercises, functional strengthening, and endurance training.

Balanced strength supports healthy movement patterns while reducing stress on healing joints and surrounding tissues.

Improving Balance and Coordination

Fractures affecting the legs, ankles, feet, or hips often reduce balance and body awareness. After spending weeks avoiding weight-bearing or walking with assistive devices, many patients feel unsteady even after the bone has healed.

At OrthoRehab, rehabilitation includes balance and coordination exercises that retrain the nervous system and improve stability. These activities help restore confidence while reducing the risk of falls and future injuries.

Improved balance also supports safer movement during everyday tasks such as walking on uneven surfaces, climbing stairs, and changing directions.

Restoring Normal Walking Patterns

Lower extremity fractures frequently alter walking mechanics. Patients may develop limping, shortened steps, or uneven weight distribution that continues long after the fracture heals.

Physical therapists evaluate gait mechanics and provide exercises that restore proper walking patterns. Gait training improves posture, stride length, weight transfer, and overall efficiency while reducing unnecessary stress on the hips, knees, lower back, and opposite leg.

Walking normally again is an important milestone toward full independence.

Regaining Functional Movement

Successful rehabilitation focuses on more than individual muscles or joints. Patients need to perform the movements required for everyday life.

Physical therapy incorporates functional exercises that simulate real-world activities such as standing from a chair, climbing stairs, carrying groceries, reaching overhead, kneeling, lifting, and walking longer distances.

By practicing meaningful movements throughout rehabilitation, patients are better prepared to return to work, hobbies, and household responsibilities.

Preventing Compensations and Secondary Injuries

After a fracture, it is common to rely more heavily on the uninjured side of the body. These compensations may temporarily reduce discomfort, but they can eventually lead to pain in other areas.

For example, favoring one leg after an ankle fracture may contribute to knee, hip, or low back pain. Protecting one arm after a wrist fracture may create shoulder or neck discomfort.

Physical therapy identifies these movement compensations and corrects them before they become long-term problems. Balanced movement helps distribute forces evenly throughout the body and supports healthier recovery.

Returning to Work and Recreational Activities

Many occupations require lifting, carrying, climbing, standing, or repetitive movements that can be challenging after a fracture. Athletes often need to regain speed, agility, strength, and endurance before safely returning to competition.

At OrthoRehab, rehabilitation programs are tailored to each patient’s lifestyle and goals. Work-specific and sport-specific exercises help prepare patients for the physical demands they will encounter after therapy.

This personalized approach helps patients transition safely from rehabilitation back to their normal routines.

Building Confidence Throughout Recovery

Many people feel anxious about using the injured body part after a fracture. Even after the bone has healed, fear of falling or reinjury may limit activity and delay recovery.

Physical therapy provides a structured progression that allows patients to rebuild confidence one step at a time. As strength, mobility, and function improve, patients become more comfortable trusting their bodies again.

Confidence is an important part of recovery because it encourages healthy movement and helps patients return to the activities they enjoy.

The Importance of Following a Personalized Rehabilitation Plan

Every fracture is different. Recovery depends on the type of fracture, the location of the injury, whether surgery was required, your age, activity level, and overall health.

At OrthoRehab, no two rehabilitation programs are exactly alike. Physical therapists continually evaluate your progress and adjust your treatment plan as healing advances. This individualized approach helps maximize recovery while protecting the healing bone throughout every phase of rehabilitation.

Recover Stronger With OrthoRehab

Healing after a fracture involves much more than allowing the bone to mend. Regaining strength, flexibility, balance, and normal movement requires expert guidance and progressive rehabilitation. Physical therapy helps patients recover safely while reducing the risk of long-term stiffness, weakness, and future injuries.

At OrthoRehab, our experienced physical therapists provide personalized care designed to help you return to the activities that matter most. Whether you are recovering from a wrist fracture, ankle fracture, hip fracture, shoulder fracture, or another broken bone, we are committed to helping you move with confidence again.

If you are recovering from a fracture or preparing to begin rehabilitation, schedule an evaluation with OrthoRehab today. Together, we can help you restore function, rebuild strength, and get back to living life with confidence.
